DepEd Official School Forms (SFs 1 – 10) Not For Sale
The Department of Education reiterated that school forms and systems that generate school forms are not for sale.
Bakit hindi dapat ipinagbili/ ibenta o pagkakitaan ang mga School Forms (SFs 1-10)?
Dahil and mga SFs na ito ay pinag-aralan at ginawa ng mga volunteers teachers, school heads, supervisors at iba pang mga kawani ng DepEd para pakibangan ng mga schools.
Walang anumang honorarium ang tinatangap ng mga boluntaryong ito – bakit nagagawang pagkakitaan ng ibang tao na wala namang contribution sa mga SFs na ito?

Generated SFs are considered final and official. Commercialized electronic school forms as mentioned in DO Np. 58, s. 2017 Section VII ( Special Provision), Shall not be recognized nor accepted. To ensure that only SFS generated from the LIS are being presented during the checking of forms, the designated LIS or ICT Coordinator is required to sign of initial each SF.

Manage or address the observed challenges. Non-compliance with the special provision of DepEd Order 58, s. 2017 (Commercialized Electronic Forms) shall also included in this report.

VII. SPECIAL PROVISION
Teachers, School heads and all concerned DepEd personnel are cautioned against individuals or groups that may attempt to sell electronic or print copies of these new forms as well as the first set of modified school forms released through DepEd Order 4, s. 2014. Teachers/ School heads may directly report such incident to the Office of the director, Planning Service at [email protected] or at telephone no. (02) 687-2744.
