DepEd School Forms 2024: Big Changes Teachers Need to Know
The Department of Education (DepEd) recently released a new memo that gives clear instructions about how to fill out school forms at the end of School Year 2024–2025. These changes are based on DepEd Order No. 10, s. 2024, which introduces the MATATAG Curriculum for Grades 1, 4, and 7.

Why Are These Forms Changing?
The changes are happening because of the MATATAG Curriculum. This is a new version of the K–12 curriculum that started in School Year 2024–2025. It was first rolled out to Grades 1, 4, and 7.
To match the new subjects and learning areas, the school forms had to be updated too. So DepEd created new versions of SF9 and SF10 for these grade levels.
Main Guidelines for School Form 10 (SF10)
1. Use the Revised SF10 for Grades 1, 4, and 7
Teachers and advisers should use the new version of SF10 that matches the MATATAG Curriculum. These are found in the memo’s annexes:
- Annex I – Grade 1
- Annex II – Grade 4
- Annex III – Grade 7

2. No Need to Re-Write Old Grades
If you’re a Grade 4 adviser, and your student came from the old K–12 curriculum, don’t worry. You don’t need to rewrite their previous grades. Just attach the old SF10 to the revised form.
- In the remarks section for Grades 4–6, write:
“See attached revised School Form 10 with learning areas based on DO 10, s. 2024.” - For Grades 1–3, write:
“Under old curriculum. See attached old School Form 10 with learning areas based on DO 21, s. 2019.”
These remarks help track which curriculum was used for a student at each grade level.
SF10 for Elementary – Now Downloadable from LIS
Teachers can now download the revised SF10 for Elementary students from the Learner Information System (LIS). This new version is specially made for Grade 4 students under the MATATAG Curriculum.
Changes in Terminologies and Status Reporting
1. No Longer Participating in Learning Activities (NLPA)
This term was used during the COVID-19 pandemic. It will now go back to “No Longer in School (NLS)” in School Form 2 and LIS.
2. Death → Deceased
The word “death” will be replaced by “deceased” in LIS records.
Senior High School Status Tags – What to Use
Senior High Schools need to tag their learners’ status correctly. Here’s how:
a. End of Semester Status
- Incomplete – If the learner didn’t meet the expectations (grade less than 75%) in at least one subject.
- Complete – If the learner passed all subjects (grade of 75% or more).
- No Longer in School – If the learner left school before the end of the semester.
b. End of School Year Status
- Regular – Learners who completed all subjects in both semesters.
- Irregular – Learners who didn’t complete the required subjects in one or both semesters.
What Should Schools Do Now?
Schools should start using the revised forms right away for Grades 1, 4, and 7. They should also inform all teachers, advisers, and records staff about the new memo.
